This should correlate with the scene numbers on your storyboard. You should add an additional line with no scene #, if the video containing the audio is completely separate from other video shots.
Location
Where will you take the video or photo? In order to be most efficient, try to do all the indoor scenes at one time and all the outdoor ones at once.

Description of the photo(s) you plan to take.
Talent
Who is going to be involved with the talking or physically be in the video or photo?
